Housing Stabilization Services (HSS) was a Minnesota Medicaid‑funded program designed to help individuals with disabilities and seniors find, transition into, and maintain stable housing to help participants live independently in the community. Frontier was actively working with 353 clients under HSS during its operation, providing support throughout both the transition into housing and the sustaining of housing stability. The program was discontinued on October 31, 2025, following a DHS review that identified concerns about program integrity. Former HSS clients were supported in connecting with alternative housing programs and community resources to continue maintaining housing stability.
