At Frontier, we believe stable housing is the foundation for independence, dignity, and long-term well-being. Through HWS-I and LTH, we work collaboratively with counties, landlords, and community partners to help individuals secure and maintain housing that supports safety, choice, and independence
Housing With Services – Independent (HWS-I) supports individuals who are able to live independently but need financial housing assistance to maintain stable housing in the community.
Participants live in scattered-site apartments, hold a standard lease, prepare their own meals, and live like typical tenants. Housing Support funding may follow the participant if they move to another approved unit.
Referrals for HWS-I, people can often self‑refer by contacting Frontier, or they can be referred by a case manager or county worker to Frontier Living. The staff then submits eligibility documentation and coordinates with the county in order to secure the Housing Support benefit.
The Long-Term Homelessness (LTH) program is a Housing Support model designed for individuals with a history of long-term homelessness who need ongoing housing assistance to achieve and maintain stability
LTH prioritizes individuals who face persistent barriers to housing and require consistent support to remain housed in the community.
Referrals for Long‑Term Homelessness (LTH) programs typically happen through the statewide Coordinated Entry System (CES). CES helps people experiencing homelessness get matched with housing programs that fit their needs, including LTH Housing Support.
Like HWS-I, LTH provides housing support only. Any supportive services must be arranged separately through appropriate programs or community providers
