We offer a wide range of support services including 24-Hour Emergency Assistance, Night Supervision, In-Home Support Services, Employment Services (Development, Exploration & Support), Integrated Community Supports (ICS), Housing Support (HWS-I & LTH), Housing Relocation (Private Pay), and HTF Housing Programs.
Our team will meet with you to understand your needs, goals, and daily challenges. Based on this, we will help you choose the service—or combination of services—that best supports your independence and safety.
Most individuals qualify if they are over 18, have a disability or condition that requires daily support, are eligible for a waiver program such as CADI, DD, EW, or BI, and have a county case manager who authorizes services. If you're unsure, we can help guide you through the process.
HWS-I is a program that helps you live independently by supporting you with budgeting, daily living tasks, community access, and maintaining housing stability. Referrals come directly from your county worker.
This service supports individuals who have experienced chronic homelessness. We help you secure stable housing, access long-term support, and maintain safe and permanent housing.
No. HSS has ended statewide. However, we continue to provide private-pay housing relocation to help with finding an apartment, completing applications, moving support, and housing transition planning.
It is a service that gives you access to help any time of day or night. You can call us for health or safety emergencies, behavioral crises, unexpected issues at home, and urgent support needs. Someone is always available to assist you.
Night Supervision provides awake staff overnight to keep you safe. This is helpful if you should not be left alone at night, need help with bathroom or mobility, experience wandering or sleep disturbances, or need reassurance or monitoring overnight.
These are supports provided in your home to help with cooking, cleaning, household tasks, medication reminders, personal care (non-medical), budgeting, going into the community, and building skills to live independently.
Our employment services help you find and keep a job in your community. Employment Development helps you find and start a job, Employment Exploration helps you discover what kind of job is right for you, and Employment Support helps you stay successful in your job.
ICS provides support in licensed apartment settings where staff are available 24/7. ICS helps with daily living and household skills, budgeting, community engagement, wellness and safety, social activities, problem solving and advocacy, and coordinating appointments and services. It's designed for adults who want independence with reliable on-site help.
Referrals typically come through your county worker. The process is: 1) Your county determines your eligibility, 2) The county sends a referral to Frontier Living LLC, 3) We schedule an intake meeting, 4) Services begin once authorized. For private-pay services, you can contact us directly.
This depends on your service type and your personal needs. Some services involve weekly visits, others may involve daily or overnight support. ICS provides 24/7 access to on-site staff. Your schedule is flexible and based on your support plan.
We do our best to match you with staff who fit your personality, needs, and preferences. If something isn't working, we can make adjustments.
